Encouraging Safe, Quality Care for Patient Choice Participants
"Centers of Excellence" is a quality-focused initiative that relies on evidence-based care to help ensure that you get safe, quality care related to certain procedures. The initiative identifies those providers with a demonstrated record of favorable outcomes for a selected procedure.
Bariatric Care. For bariatric care, including surgery for morbid obesity, the designated
Centers of Excellence have been approved by the American Society of Bariatric Surgery
(ASBS) and the Surgical Review Corporation (SRC).

Please note:
- The list of providers in the Patient Choice Centers of Excellence program may periodically be updated, so be sure to re-check the list periodically for the most up-to-date information.
- If you are a Patient Choice member, you should verify that a particular provider listed is in-network for your program/Care System. Not every Centers of Excellence provider is considered in-network for every program/Care System offered by Patient Choice.
- If you are a Patient Choice member, you may verify if a provider is participating in your program by using the online provider directory or by contacting Customer Service for assistance at the phone number listed on the back of your ID card.
- Patient Choice members should always verify coverage prior to receiving services by reviewing your Summary Plan Description or other coverage documents, or by contacting Customer Service for assistance at the phone number listed on the back of your ID card.
