Patient Choice creates and manages patient-focused health
care networks. The company partners with a number of administrators to do
claims processing and customer service for its care system programs, including such administrators as UMR (formerly Fiserv Health) and
Meritain Health (formerly CBSA), among others.
If you have questions about your plan including medical eligibility,
benefits or claims, you should contact your program administrator. Contact
information for your program administrator is listed on the back of your
medical identification card.
If you are looking to enroll in Patient Choice and have not received a
medical identification card, check your employer's enrollment material for
the appropriate contact information.
